- On the Please select business unit prompt, choose Health Care Quality & Compliance, then select OK.
- In the lower-left section labeled LICENSE VERIFICATION, FACILITY LOCATOR & INSPECTION REPORTS, select Click Here.
- On the Licensee Search page, set Business Unit to Health Facilities. Entity Type should populate as Agency.
- Enter 11188 in Credential Number.
- Set Credential Type to HOSPICE CARE – PROGRAM OF CARE, then select Search.
- Open the result for QUALITY OF LIFE HOSPICE LLC. The license may display as 11188-HPC-3. Select View Detail and confirm the current status, the Las Vegas address, and Federal Provider Number 291672.
