Hospice Referrals in Las Vegas
Quality of Life Hospice partners with attending physicians, hospital discharge planners, medical social workers, and post-acute case managers throughout the Las Vegas Valley to deliver rapid, seamless palliative and hospice admissions for patients with advanced illness.
Rapid Bedside & Home Response: We conduct patient bedside evaluations within 2 to 4 hours of referral receipt, coordinate DME and medication delivery before discharge, and maintain open communication with the patient’s primary medical team.
Clinical Indicators for Hospice Appropriateness
Under Medicare guidelines (CMS-418), patients qualify for hospice care when two physicians certify a life expectancy of six months or less if the terminal illness runs its normal course. General decline markers include:
Functional Decline
Palliative Performance Scale (PPS) or Karnofsky Performance Scale (KPS) ≤ 50–60%. Progressive dependency in ≥ 3 of 6 Activities of Daily Living (ADLs).
Nutritional Compromise
Unintentional weight loss > 10% in past 6 months or > 5% in past 3 months. Serum albumin < 2.5 g/dL. Progressive dysphagia or aspiration history.
Acute Utilization
Multiple ED visits, ICU admissions, or hospital readmissions within past 6 months. Escalating symptom burden despite optimized medical therapy.
Shift to Comfort Goals
Patient or surrogate expresses preference to avoid further aggressive interventions, intubation, dialysis, or invasive ICU treatments in favor of palliative comfort.
