Clinical Indicators for Hospice Appropriateness

Under Medicare guidelines (CMS-418), patients qualify for hospice care when two physicians certify a life expectancy of six months or less if the terminal illness runs its normal course. General decline markers include:

Functional Decline

Palliative Performance Scale (PPS) or Karnofsky Performance Scale (KPS) ≤ 50–60%. Progressive dependency in ≥ 3 of 6 Activities of Daily Living (ADLs).

Nutritional Compromise

Unintentional weight loss > 10% in past 6 months or > 5% in past 3 months. Serum albumin < 2.5 g/dL. Progressive dysphagia or aspiration history.

Acute Utilization

Multiple ED visits, ICU admissions, or hospital readmissions within past 6 months. Escalating symptom burden despite optimized medical therapy.

Shift to Comfort Goals

Patient or surrogate expresses preference to avoid further aggressive interventions, intubation, dialysis, or invasive ICU treatments in favor of palliative comfort.

  2. Continuous Home Care: Intensive nursing support (up to 24 hours daily) during acute symptom crises to achieve stability and prevent unnecessary rehospitalization.
  3. General Inpatient Care (GIP): Short-term inpatient management in an acute contracted medical facility for severe, intractable pain or symptom crises requiring round-the-clock RN oversight.
  4. Inpatient Respite Care: Up to 5 consecutive days of inpatient relief at a Medicare-approved facility to prevent caregiver burnout and support family stability.

Physician Collaboration & Continuity of Care

Referring physicians may choose to remain the primary attending physician throughout the hospice journey or delegate primary hospice oversight to our Board-Certified Hospice Medical Director. We provide regular status updates, notify your office of any medication adjustments, and ensure continuous clinical compliance.
